Steißbeinfisteln operativ entfernen

Früher wurden Steißbeinfisteln standardmäßig mit einer Operation, der sogenannten Lappenplastik behandelt. Heute werden Behandlungsoptionen wie das Pit-Picking bevorzugt, da sie minimalinvasiv sind und weniger umliegendes Gewebe in Mitleidenschaft ziehen. In unserer Praxis führen wir die Lappenplastik in modifizierter Form nur noch durch, wenn es durch Voroperationen zu einem größeren Defekt der Mittellinie gekommen ist.

Die klassische Methode, um eine Steißbeinfistel zu behandeln, ist nach wie vor eine Operation. Dabei wird das Fistelgewebe spindelförmig herausgeschnitten. Ein einfacher Nahtverschluss ist aufgrund der Scherkräfte, die in der Pofalte wirken, selten erfolgreich. Wenn die Wunde nicht offen ausheilen soll, muss man eine Lappenplastik anwenden. Dabei wird körpereigenes, gesundes Gewebe benutzt, um die Wunde zu verschließen. Mit dieser Technik kann die Wundheilung erheblich beschleunigt werden. Außerdem lässt sich so die Gefahr eines Rezidivs senken.
Nachteil dieser Technik ist, dass ein größerer Schnitt notwendig ist, um genügend Gewebe für den Wundverschluss zur Verfügung zu haben. Wenn es zu einer Wundheilungsstörung kommt, muss die Wunde in vielen Fällen trotzdem offen ausheilen und gesundes Gewebe wurde unnötig in Mitleidenschaft gezogen.

Lappenplastik nach Dr. Hofer
Ist es durch wiederholte Voroperationen der Steißbeinfistel zu einem größeren Defekt der Mittellinie gekommen, wenden wir eine von Dr. Hofer weiterentwickelte Version der Lappenplastik an. Als erstes werden Narbe und die erneut aufgetretene Fistel entfernt. Anschließend wird die Wunde mit einer Naht seitlich der Mittellinie versorgt, da sich gezeigt hat, dass diese Technik die besten Heilungschancen mit sich bringt. Blut- und Lymphgefäße werden bestmöglich geschont, wenn das Gewebe für den Wundverschluss aus der Unterhaut entnimmt. So können außerdem Blutergüsse und Ansammlungen von Lymphflüssigkeit unter der Wunde vermieden werden.
Bei dieser Schnittführung kann in der kritischen Zone am Unterrand der Narbe die weiter zur Seite verlagert werden:

Der-Haut-Unterhaut-Lappen wird spannungsfrei eingenäht, der Defekt der Mittellinie ist repariert. Deutlich zu erkennen ist die Abflachung der Gesäßfalte und die seitliche Nahtlinie, beides Faktoren, die einem Rezidiv des Sinus pilonidalis vorbeugen.

Ablauf der Behandlung
Die Operation wird meist in Vollnarkose durchgeführt, eine örtliche Betäubung ist in Einzelfällen auch möglich. Körperliche Schonung ist danach für zwei bis drei Wochen erforderlich, in dieser Zeit sollte man nicht lange sitzen und keinen Sport treiben. Die Hautfäden werden nach zwölf Tagen gezogen. Die tiefen Fäden lösen sich von selbst auf.
Die Patienten kommen nach dieser Operation wöchentlich zur Kontrolle in unsere Proktologische Praxis in München. Kleinere Wundheilungsstörungen sind häufig, aber durch entsprechende Pflege der Wundränder, Entfernung von Nahtmaterial und spezielle Wundverbände gut in den Griff zu bekommen.
